Background
Efforts to broaden ongoing support for regional population health strategies
across Kimberley ACCHSs became possible with national ear/health funding
in late December 1996. Until this point, KAMSC regional population health
support was only available through the KAMSC Medical Director position.
Since then, KAMSC has also successfully attracted national initiative
funding targeting eye health and indigenous sexual health care.
These individual 'body part' funding arrangements are made on a grant
by grant basis annually, with individual reports and specific performance
indicators.
From within this funding framework, KAMSC endevours to provide support
for comprehensive and holistic population health care within the community
controlled primary health care services within the community.
Role and Responsibility
The roles and responsibilities of the Population Health Unit are
diverse. They include to:
• Promote
full utilisation of patient information and recall systems that
support integrated
comprehensive
care, opportunistic and targeted screening
and evidence based management of common health problems.
• Assist
development and support uptake of evidence based health care
plans and management protocols.
• Coordinate rotation
of shared resources and support full utilisation equipment
across the region.
• Contribute to AHW
student training/staff development and orientation/evaluation
within the
region.
• Perform regular assessments
of population health activities undertaken by ACCHS’s, to identify
effective work practices and areas where problems need addressing.
• Advocate for the role
of AHW’s
within the provision of population health care.
• Prepare reports on
unit activities for KAMSC, funding sources and contribute
to submission / budget
preparation.
• Assisting
ACCHS’s to target and deliver population health
programs.
